Hospital to Home FAQs

Will you work with other healthcare professionals who are involved in my care?

We will work with other healthcare professionals involved in your care. We can liaise directly with your Discharge Teams prior to your hospital discharge to ensure that everything is prepared in time for the moment you leave hospital. We can work with your GP and Social Worker to ensure that we fully understand your current condition and circumstances.

What happens if I do not have a confirmed discharge date?

If you don’t have a confirmed discharge date, we can still arrange your care service. A member of our Care Team can visit you in hospital or at home and discuss your care requirements at a date and time to suit you. We will carry out a risk assessment of your home to ensure your safety. This can be arranged with family or friends, or for a date when you have been discharged from hospital. We can often complete the assessment on the day that your care is due to commence. We will then arrange for suitable Carers to provide your healthcare service with whom you can meet prior to your service commencing.

Will I need a Nurse, Healthcare Assistant or Support Worker to provide my care?

To determine whether you will require the support of a Nurse, Healthcare Assistant or Support Worker we will carry out a full care assessment. Carers are able to support you with everyday activities such as getting in and out of bed, however, a Nurse will support you with more complex requirements such as dressing wounds and administering complex medication. Should you wish to have support from a Nurse, Healthcare Assistant and Support Worker at varying times, we will be more than happy to arrange this for you.

Can I reduce the amount of care that I receive if my condition improves?

Our care service is flexible, allowing you to change the level of support that you receive in-line with your requirements. We aim to actively support your rehabilitation progress to encourage you to live as independently as possible, as quickly as possible. As your condition improves your support needs are likely to reduce and the service that we provide can easily be adapted to the level of care that is appropriate for you.
